Understanding Supplier Forms

Supplier forms are available for download from the Supplier Portal. When downloaded, a fillable pdf version is provided. The supplier completes the required fields on the form, saves a copy to their hard drive and then uploads the saved form to submit. The data will be encrypted and sent to the state’s Supplier Operations team at OBM Shared Services for processing. The following information is offered for understanding the terms used in processing the supplier operations forms.

Definition of Taxpayer Identification Number (TIN)

A Taxpayer Identification Number (TIN) is an identification number used by the Internal Revenue Service (IRS) in the administration of tax laws. It is needed either by the Social Security Administration (SSA) or by the IRS. A Social Security Number (SSN) is issued by the SSA whereas all other TINs are issued by the IRS.
